She is his mother!....a delusional version of her, that is.
At the end of the episode as they chatted in the doctor's waiting room, he asked her, "Why do you look like my mother?" and she answered, "I'm the part of you that's wants you to get better. Who else would I look like?"
I almost missed that...sometimes I miss what he says...I had to replay it and pay closer attention to catch it all.

That was the cliffhanger, Dipo...the show ended with Sherlock at a doctor's office getting a brain scan.
My guess (pure speculation) is that it turns out to be some kind of benign brain tumor or maybe an aneurysm (something very dangerous and scary but possibly very curable with surgery) and then next season will be about whether he loses his brilliant gift of deduction. And then he'll get better...back to his old weird self.
Btw, I'm glad his hair is back....but I wonder if he'll shave it again if he needs brain surgery.

Since we have a lot of personal experience with dementia in my family (my mom), I was wondering if he has Alzheimers or some other form of dementia. I also figured out he was having memory issues (when he couldn't remember what was in the boxes), and that the woman was a figment of his imagination (since the basement office wasn't burned). For a while there, I was with you, Dipo, wondering if we'd missed a show.
Does anyone know if this show has been renewed for next year? I was ready to let it go and prepared myself for cancellation, but they can't just leave us hanging like this!

Loved the episode. Sherlock makes me laugh then cry throughout the show. 💗
The plastic at the beginning made me think of Dexter and now we have Desmond Harrington as a new character this season. Oooh, the end.  I had a feeling when he was out of breath on the phone.

Suz, he / doctor's think it may be that brain issue that happens when you have too many concussions, like the football players have gotten. So he has to let his brain heal.
However, I think it is something else.
